Health Education Program

Health education is available to IHC medical members that have Diabetes or other chronic conditions such as hypertension. Having these conditions can be complicated and our health educators are here to teach you about the condition, how to manage it, and set goals with you to improve your health and well being. If you want to quit smoking, our health educators can also help you with that. Heath educators can see you before or after your medical appointments, or you can make a separate appointment to see the health educator. You can also receive health education over the phone.
The Diabetes Education and Empowerment Program is a 6-week group education program for IHC medical patients that are at risk for diabetes or have diabetes. It is a fun, interactive program gives you a better understanding of diabetes and how to manage it.

Program Eligibility

All IHC members are eligible to receive Health Education. A provider referral is not necessary.
