Welcome to our clinic, the AI/AN Patient Relations team members are Anita Carmona, Paulette Moore and Stephanie Reynolds.
Welcome to The Indian Health Center of Santa Clara Valley (IHCSCV). The Medical Department has expanded our AI/AN Patient Relations position to a team of three employees to help with better access for our patient needs.

• Call us for same day appointment availability or for future appointments.
• We can assist you in the following:
• Checking, cancelling or rescheduling any appointments.
• Contacting or messaging our medical staff members.
• Directions and finding any of our five sites.
• Starting your family tree and become your own family historian.
• Contacting your state and federal tribes.
• Help with resources in our local AI/AN community to assist your needs
• Provide fliers for upcoming events and powwows for the community.
• Conflict resolution

• When you come to IHCSCV, the AI/AN - Patient Relations Team is here to assist you. Throughout your care, we act on your behalf and make your needs a priority. We are your personal liaisons.
• At the IHC you are not just a number.
• We understand your needs and want to make your appointment in a timely manner.
